Pass certificate re-issue application preparation

1. We will prepare a certificate of reissuance application. If you need a copy for the applicant, please submit two copies.
2. In each column of the re-grant application form, please describe the same contents as the corresponding column of the certificate. If you do not know the contents, please contact the Testing and Examination Department.
3. Please specify the “Reissue Reason” column specifically.
4. If the applicant’s or (or) manufacturer’s company name or (and) address has changed, please apply for a certificate of re-issuance first. Please apply for the change of items after re-issuance of the certificate. In this case, please describe the current contents in the applicant or (and) manufacturer column of the regrant application form.

Documents to attach to re-grant application

1. If you still have a damaged certificate, please attach it.
2. If you lose the “Same Model List”, “Attached Drawings List” and “Summary of the Manufacturing Inspection Equipment, etc.” attached to the certificate, all the items with the same number and contents as those attached It is necessary, please create a new copy and attach one copy each. However, if it is difficult to create them due to loss etc., it is possible to create using scanned data saved by our association, so please consult in advance.
