    Critical parts for type of protection (PCB documents for type “i”, glass window dimension for type “d”, etc.)Marking*
    
  • Contact information of applicant and agent
  • Explanatory description of type identification codes
  • Certificate of conformity
  • Test report prepared by the manufacturer or issued by third party
  • Description with respect to performance of the equipment and description regard to operation of the equipment*
    For example, user manual (instruction manual) *
  • Description of reasons for acceptable variants
  • Indication of how to return the application counterfoil and the certificate
    Note. Other than domestic shipment, the charge for the delivery is requested separately.
  • What TIIS can confirm a payment such as a copy of bank transfer statement
