• Two pieces of application form for alteration of content in certificate filled out (One for counterfoil) *
• The whole set of the certificate (It shall be original hard copy)
• A document verifying that the company name / address has been changed
• Difference between old and new
• Authorized document which proofs the changes issued by an administrative organ
• Schedule of Variants (in case of modification of type identification codes)*
• Contact information of applicant and agent
• Indication of how to return the application counterfoil and the certificate
  Note. Other than domestic shipment, the charge for the delivery is requested separately.
• What TIIS can confirm a payment such as a copy of bank transfer statement

*: Critical document should be prepared in Japanese.
