• Two pieces of application form for reissuance of certificate filled out (One for counterfoil) *
• The whole set or part of original certificate (if exists)
• Electronic data of schedule of variants (if applicable) *
• Electronic data of list of drawings *
• Electronic data of document about manufacturing and inspection facilities, etc. of manufacturer (if applicable) *
• Contact information of applicant and agent
• Indication of how to return the application counterfoil and the certificate
  Note. Other than domestic shipment, the charge for the delivery is requested separately.
• What TIIS can confirm a payment such as a copy of bank transfer statement
